• Check Fraud Claim - Describe the check fraud claim in detail on this affidavit.
• Notification of Fraudulent Debit Card Transaction - In the event you have experienced an unauthorized charge on your UCU Debit Card.
• Notification of Disputed Debit Card Transaction - If you are unable to resolve a disputed debit card transaction with a merchant, please fill out this form. 
• Notification of Disputed Mastercard Transaction - If a Mastercard transaction was unauthorized, please fill out this form.
• Mastercard Authorized User - Authorizing another user for your Mastercard can be done easily by filling out this form.
• Mastercard Automated Payment Service - If you would like to set up automatic payments from your Mastercard at a minimum, specified, or remainder amount, please fill out this form.
